WHO: U.S. Department of Agriculture & Food and Drug Administration/span> WHAT: According to congressional testimony by the General Accountability Office, USDA inspectors knew about unsanitary conditions at an Iowa farm involved in a massive egg recall, but did not inform fellow inspectors at the Food and Drug Administration, which is supposed to guarantee egg safety.

WHY IT’S AN OUTRAGE: Since 2003, USDA and FDA have conducted duplicate inspections at 1,451 domestic food-processing facilities, yet both agencies somehow missed the salmonella contamination that led to the recall of 380 million eggs.
